REQUEST FOR SEARCH OF SAFE DEPOSIT BOX FOR WILL OR BURIAL INSTRUCTIONS

             Pursuant to 755 ILCS 15/1, I,                                                    , hereby request and authorize any qualified bank officer or employee to search safe deposit box number              , rented in the name of                                                       , deceased, for the Will and burial document s of the decedent.  The decedent died  on                                        , and a certified copy of the death certificate is attached.

             I understand that the search is to be conducted by the bank in my presence, that any apparent original Will and codicils found will be filed with the appropriate court, and that any burial documents may be given to me.  I understand that no other contents will be removed.

             I,                                                                          , being duly sworn, hereby state:

 1. I believe the box may contain the original Will or burial documents of the decedent.

 2. I am interested in the filing of the Will of the decedent or in the arrangements for the burial of the decedent and I am an "interested person" pursuant to statute as indicated below:
